I understand the fees. Registration fee: $350. Tour Pack: Jersey/socks- $50(to be purchased at time of 'registering'). Scout Central- $22.99/month. Player must have an active Scout Central player account(www.scoutcentralhockey.com) no later than 30 days prior to the event. Account may be canceled after the event if the player wishes to do so.

I understand that Swan Hockey makes no representation as to which division(s)/level(s) of the tournament Swan Hockey will compete in. Swan Hockey may also field multiple teams(possibly in different divisions/levels) and will assign the player to a team.

I understand that there are many risks inherent in and incidental to participation in the Swan Hockey ‘Program’ . I assume all risks inherent to such participation and hereby release, absolve, indemnify and hold harmless the Swan Hockey LLC, its partners, affiliates, sponsors, officers, employees, and staff of any claim arising out of an injury(including death) to me (or the applicant, if different from the undersigned) requiring professional medical attention. I hereby authorize and request Swan Hockey LLC, its partners, employees, and staff to act on my behalf under prevailing circumstances in the event of an injury or in the event that I am not able to act for myself or cannot be reached.
